Health & Liability Consent

This document constitutes the informed consent agreement between the client and CasaBliss, Inc. By checking the consent box during booking, the client acknowledges having read, understood, and agreed to the terms herein.

CasaBliss provides professional massage therapy services in the client's location of choice. Services are performed by Florida-licensed massage therapists. Massage therapy is not a substitute for medical diagnosis or treatment.

The client agrees to disclose any relevant health conditions, injuries, medications, or contraindications prior to the session. These include but are not limited to:

• Pregnancy (especially first trimester)
• Blood clots, varicose veins, or circulatory disorders
• Open wounds, skin infections, or recent surgeries
• Fever, contagious illness, or immune disorders
• Cancer (active or in treatment)
• Severe osteoporosis or bone disorders

Failure to disclose relevant health information releases CasaBliss from liability for adverse reactions.

CasaBliss therapists are trained and licensed massage therapists. They are not medical doctors and will not diagnose conditions, prescribe medication, or perform chiropractic adjustments. Any discussions about health conditions are for informational purposes only.

The client acknowledges that massage therapy, while generally safe, carries some inherent risks including temporary soreness, bruising, or aggravation of pre-existing conditions. The client voluntarily assumes these risks.

The client has the right to stop the session at any time and to refuse any techniques. The therapist reserves the right to decline or stop service if the client exhibits signs of a contraindicated condition, is intoxicated, or behaves in an inappropriate or unsafe manner.

CasaBliss and its therapists shall not be liable for any direct, indirect, incidental, or consequential damages arising from massage therapy services, including but not limited to physical injury, property damage, or adverse health reactions, except in cases of gross negligence or willful misconduct.

All health information shared by the client is kept strictly confidential and used only to provide safe, appropriate massage therapy. It is accessible only to your assigned therapist and CasaBliss management.

By proceeding with a booking and checking the consent checkbox, the client confirms that they are 18 years of age or older (or have parental/guardian consent), have read this document, understand its contents, and voluntarily agree to its terms.
