Health & Liability Consent

Effective: January 1, 2025

Purpose

This document constitutes the informed consent agreement between the client and Casa Bliss Massage LLC. By checking the consent box during booking, the client acknowledges having read, understood, and agreed to the terms herein.

Nature of Services

Casa Bliss Massage LLC provides professional massage therapy services in the client's location of choice. Services are performed by Florida-licensed massage therapists. Massage therapy is not a substitute for medical diagnosis or treatment.

Client Health Responsibility

The client agrees to disclose all relevant health conditions, injuries, medications, symptoms, recent procedures, and known or suspected contraindications before the session. This includes contagious and non-contagious conditions that could affect the safety or suitability of massage. These include but are not limited to:

  • Pregnancy (especially first trimester)
  • Blood clots, varicose veins, or circulatory disorders
  • Open wounds, skin infections, or recent surgeries
  • Fever, contagious illness, or immune disorders
  • Cancer (active or in treatment)
  • Severe osteoporosis or bone disorders
  • Any other known or suspected health condition, whether contagious or non-contagious, that could affect the safety, comfort, or suitability of the massage session

Failure to disclose relevant health information releases Casa Bliss Massage LLC from liability for adverse reactions.

Scope of Practice

Casa Bliss Massage LLC therapists are trained and licensed massage therapists. They are not medical doctors and will not diagnose conditions, prescribe medication, or perform chiropractic adjustments. Any discussions about health conditions are for informational purposes only.

Assumption of Risk

The client acknowledges that massage therapy, while generally safe, carries some inherent risks including temporary soreness, bruising, or aggravation of pre-existing conditions. The client voluntarily assumes these risks.

Right to Refuse or Stop Service

The client has the right to stop the session at any time and to refuse any techniques. The therapist reserves the right to decline or stop service if the client exhibits signs of a contraindicated condition, is intoxicated, or behaves in an inappropriate or unsafe manner.

Professional Boundaries & Zero-Tolerance Misconduct Policy

Massage therapy services provided by Casa Bliss Massage LLC are strictly professional and therapeutic in nature. No service offered by Casa Bliss Massage LLC includes sexual activity, sexual contact, sexual favors, sexual touching, sexual conversation, or sexual solicitation of any kind. The therapeutic relationship must remain professional at all times.

Prohibited Client Conduct

The following conduct is strictly prohibited and will not be tolerated:

  • Sexual comments, jokes, requests, propositions, or advances
  • Indecent exposure or intentional exposure of genital areas
  • Requests for inappropriate touching or for sexual services
  • Attempts to touch the therapist
  • Physical intimidation, verbal harassment, or threatening behavior
  • Any conduct that causes the therapist to feel unsafe, threatened, harassed, intimidated, or uncomfortable

Therapist Safety Rights

Every therapist working with Casa Bliss Massage LLC has the right to maintain professional boundaries, to feel safe while providing services, to refuse inappropriate requests, and to leave the premises immediately if safety concerns arise.

Immediate Session Termination

Therapists may provide a verbal warning at their sole discretion but are never required to do so. Casa Bliss Massage LLC reserves the right to immediately terminate a session, without warning, whenever a client's behavior is determined to be sexual, threatening, abusive, harassing, unsafe, or otherwise inappropriate. This protection is absolute, and therapist discretion is fully preserved.

Payment Enforcement

If a session is terminated due to inappropriate client conduct, the client remains financially responsible for the entire booked session. The full session fee remains due, any deposit is non-refundable, applicable membership benefits or session credits may be forfeited for that session, and no refunds will be issued.

Future Service Refusal

Casa Bliss Massage LLC reserves the right to permanently refuse future services to any client who violates these professional conduct standards, including permanent account and booking restrictions and the refusal of future appointments.

Incident Documentation

Casa Bliss Massage LLC may document incidents involving inappropriate conduct for internal safety, risk-management, compliance, and therapist-protection purposes. When appropriate, such incidents may be reported to law enforcement authorities.

In-Home Visit Protection

Because Casa Bliss Massage LLC therapists often provide services within private residences, the client agrees to maintain a safe, respectful, and professional environment for the duration of the visit. A therapist may immediately leave the premises if safety concerns arise, if harassment or threatening behavior occurs, or if professional boundaries are violated.

Limitation of Liability

Casa Bliss Massage LLC and its therapists shall not be liable for any direct, indirect, incidental, or consequential damages arising from massage therapy services, including but not limited to physical injury, property damage, or adverse health reactions, except in cases of gross negligence or willful misconduct.

Privacy of Health Information

All health information shared by the client is kept strictly confidential and used only to provide safe, appropriate massage therapy. It is accessible only to your assigned therapist and Casa Bliss Massage LLC management.

Acknowledgment

By proceeding with a booking and checking the consent checkbox, the client confirms that they are 18 years of age or older (or have parental/guardian consent), have read this document, understand its contents, and voluntarily agree to its terms.
